Before your visit: to help make your visit as smooth and efficient as possible, please come prepared with the following
  • You should receive a text message with a link to check-in 2 days prior to a scheduled visit. Please try to complete this before arriving. If you don’t get the link, you will be asked to scan a QR code and complete before being seen. 
  • Bring a valid photo ID and your insurance card, if applicable. 
  • Prepare a list of health concerns and questions you’d like to discuss. If you’re managing an ongoing condition, note any recent changes in symptoms. 
  • Be ready to share your medical history, including allergies, family health history, and any past hospitalizations or surgeries. 
  • Bring a list of your current medications, including dosages, how often you take them, and the prescribing provider. 
  • If available, bring copies of recent medical records, including immunizations and lab results. 

During your visit 
Your visit is a partnership between you and your care team. To get the most out of your appointment: 

  • Review your questions and concerns with your provider. Taking notes can help you remember next steps and care instructions.
  • Answer questions openly and honestly about your health, lifestyle, and habits, including diet, exercise, and use of tobacco, alcohol, or drugs.
  • Let your provider know if you are pregnant or planning to become pregnant, as some treatments may not be appropriate.
  • Ask questions about your condition, treatment options, and how to manage symptoms.
  • Discuss any new medications, including how to take them and possible side effects to watch for. 
Registration Forms
New patients can save time at their first appointment by completing these forms in advance. You should receive an SMS with a link to complete an eCheck-in 2 days prior to a scheduled visit. You do not need to do both sets of forms.  If you can’t complete the forms electronically, you can download and print the documents to complete them before your visit.

Medical Records Release
Authorizes the release of your health information in accordance with federal and state privacy laws. This form is required to request or share your medical records.
Mammography Images Release
Allows the release of your mammography results to other healthcare providers upon request.
Health Information Exchange (HIE) Opt-Out Form
Health Information Exchanges allow your Sanitas care team to securely view your medical information electronically. Use this form if you choose to opt out of HIE participation

Yes. You can bring family members with you for your own medical appointments. Your family members (kids, adults and seniors) are also welcome to become patients.
To help us make the most of your first visit, please bring: A valid photo ID (driver’s license, passport, or other government-issued ID), your insurance card (and/or Medicare or Medicaid card if applicable), a list of your current medications and supplements (or bring them with you), information about your medical history (past surgeries, chronic conditions, allergies, vaccination records), any recent medical records (such as lab results, imaging, or specialist notes), a list of questions or concerns you want to discuss.
