Our Chronic Care Program is designed to support you physically, emotionally, and socially. We match you with a care team—including your doctor, coaches, and educators—who work together to help you manage your condition, prevent complications, and achieve your health goals.

With 24/7 virtual access to care, medication support, nutrition counseling, stress management, and more, we make it easier to stay on track. You’ll have access to in-person and virtual check-ins, group classes, and referrals to mental health or social support services. For eligible patients, care-at-home services are also available.

Who can enroll in a Chronic Care Program?
Any Sanitas patient 18 or older with a qualifying chronic condition—such as diabetes, heart disease, COPD, asthma, hypertension, or those on long-term anticoagulation—may be eligible. Your care team will determine if the program is right for you based on your medical needs and goals.

What services are included in the program?
Your care team—including your doctor, care coach, and educators—will work together to provide:

  • Labs and diagnostic testing

How long does the program last?
There’s no one-size-fits-all answer. Most patients are evaluated every six months to assess progress. If health goals are met, you may graduate from the program—but you can re-enroll at any time if your needs change.
