Obtaining your medical records
If you wish to receive a copy of your medical records, or your child’s/ward’s medical records, you may do so by contacting Sanitas USA, Inc. by faxing your request for medical records to 305-402-0855. You can also give us a call to 305-894-4291 if you have any further questions.
Sanitas USA, Inc. will continue to maintain your records as legally required. While Connecticut law permits providers to charge for copies of medical records, we will provide such records to you free of charge.
