Urgent care visits are only valid at Sanitas Medical Center in your state of membership (Florida). |
| Behavioral Health | One (1) initial consultation and one (1) follow-up visit with a clinical social worker for mild to moderate behavioral health issues (such as mild to moderate anxiety and/or depression) per calendar year. Behavioral Health services require a referral by a Sanitas PCP as the result of a PCP visit at a Sanitas Medical Center. |

***We may periodically change the terms of the SanitasCare Direct Primary Care (DPC) Program to comply with applicable laws (that change from time to time), and/or to otherwise ensure the viability of the DPC Program so that the valuable services continue to be available to our members. If we deem any changes to this Agreement necessary, such as adjustments to the included services and/or membership fees, we will provide you with written notice at least 60 calendars day prior to the effective date of any material change(s). If you disagree with any changes, you have the right to terminate your membership by providing 30 days’ notice.
